Applications are open to people aged 18 years and over who have an assessed care and support need from their Local Authorities’ Adult Social Care Team, and would benefit from living independently with support available when required. This may include:
- Older people who need additional support
- Adults with physical disabilities
- People living with dementia
- People with sensory support needs
- People with mental health support needs
- Residents who may otherwise struggle to remain independent at home
- Adults with learning disabilities
Applicants who do not currently live in South Tyneside will also need to evidence that they have a local connection to South Tyneside.
Extra Care schemes work best when there is a balanced mix of residents with different levels of care and support needs.
All applications are considered carefully by a Lettings Panel to make sure the scheme remains safe and suitable for everyone.
Eligibility Criteria
Diagnosis and Support Needs:
Applicants must have an assessed care and/or support need. The scheme is specifically designed to cater for people who require care and/or support, have physical disabilities or dementia.
Age and Residential Status:
The scheme is open to individuals aged 18 years or older. Priority may be given to people who are at risk of going into an institutional setting and who are able to live independently and those with a housing need.
Local Connection:
Preference will be given to applicants who have a local connection to the area or have a compelling reason to live in the specific community served by the Scheme. Local connection can be established through family ties, previous residence, or existing support networks in the area:
- Being a resident of permanent accommodation in South Tyneside for at least 6 months out of the last 12 months, or 3 years out of the last 5 years
- Being in employment within the borough
- Having 'close family' living in South Tyneside for at least the previous 5 years. Close family means, parents, grandparents, children, siblings, stepparents, step children, grandchildren or adoptive parents and children. In exceptional circumstances and where support is required, 'close family' may be extended to aunts, uncles and in laws
Exceptions to the Local Connection Criteria are listed below:
- Former members of the Armed Forces
- Serving members of the Armed Forces who need to move because of a serious injury, medical condition or disability sustained as a result of their service.
- Serving or former members of the Reserve Forces who need to move because of a serious injury, medical condition or disability sustained as a result of their service.
We may ask you for the name, address, date of birth and relationship of the person, alongside evidence that they live in South Tyneside (such as a Council Tax bill or letter).
Support Requirements:
Applicants must have a documented care and/or support need which can be accommodated by the Scheme and by the on-site care provider to enable the Applicant to live there independently with appropriate support services provided.
Applicants are typically required to undergo an assessment by a dedicated social worker
Compatibility and Suitability:
The scheme aims to create a supportive and harmonious living environment. Compatibility with existing or potential tenants is considered during the nomination process
Suitability is assessed based on factors such as the individual's ability to adapt to the setting and willingness to participate in care and support.
Applicants' needs or behaviour must not pose a risk to other residents or their care provider or the Association' staff at the scheme.
